This form collects school demographics, contact information and school organization. This information will be used to maintain the School Contacts web page.

All 1601 updates should now be completed through the new Education Data Exchange (EDX). Schools and districts can edit their details and contact information directly in the system, eliminating the need to submit forms.

Instructions
1. Visit the September K-12 Data Collection web page for the 1601 instructions.

2. Update your information on EDX.
