We believe strongly in empowering people with knowledge and with concrete skills to apply it in their context. CCMI\u2019s skill-building workshops include:<\/p>\n<ul>\n<li>Brief Action Planning<\/li>\n<li>Health Literacy<\/li>\n<li>Cultural Competence<\/li>\n<li>Shared Decision Making<\/li>\n<li>Motivational Interviewing<\/li>\n<li>Patient Engagement<\/li>\n<li>Social Determinants of Health<\/li>\n<li>Brief Interventions.<\/li>\n<\/ul>\n<p><strong>Implementing strategies for system-wide change:<\/strong><br \/>\nCCMI has consulted and coached systems across Canada, the U.S. and internationally, providing quality improvement support for system redesign aimed at integrating self\u2010management support techniques into patient care. Our expertise include:<\/p>\n<ul>\n<li>chronic disease management,<\/li>\n<li>care coordination, and<\/li>\n<li>patient\u2010centered medical home.<\/li>\n<\/ul>\n<p><strong>Facilitating collaboration in new ways:<\/strong><br \/>\nOur work is premised on a person\u2010centered, population\u2010based approach that ensures the system of care addresses its community\u2019s needs wherever they are with managing their health.
